Caddie Conduct and Report Form

Conduct

If a caddie or spectator becomes disruptive to the point of receiving a formal complaint from an official, volunteer, competitor or caddie, he or she will be given a warning upon validation by U.S. Kids Golf. Caddie will sign acknowledgment form stating he/she has received this warning.

After a second offense, the caddie will be placed on probation and will be required to pass the Positive Coaching Alliance Second Goal Parent online course before participating in future events. Caddie will sign acknowledgment form stating he/she has been placed on probation.

Upon a third offense, caddie privileges will be suspended for 6 months. Caddie will sign acknowledgment form stating he/she has been suspended.

Repeat offenders may be subject to an indefinite suspension of caddie privileges at the discretion of U.S. Kids Golf.

Violations include profanity, abusive language, club throwing, cheating, disrespect of volunteers, competitors, or officials, abuse to the golf course or the facilities, littering, or any other conduct unbecoming of a caddie. U.S. Kids Golf reserves the right to dismiss any caddie at any time for an egregious offense.
